Consumer Health Data Privacy Policy

Introduction
Salix Compass, a project of Evergreen Social Impact, is committed to protecting your consumer health data privacy in Washington State. This policy supplements our standard Privacy Policy and applies specifically to "Consumer Health Data" as defined by the Washington State My Health My Data Act (MHMDA).
Important: At this time, Salix Compass does not provide direct patient services and does not collect or process consumer health data or protected health information (PHI) through our public website or the Weaver Crest platform. Any references in this policy describe how we will handle such data once those services launch.
1. Consumer Health Data We Collect
If, in the future, we provide direct navigation services to individuals, we may collect "Consumer Health Data," which is information linked to your past, present, or future physical or mental health status under Washington MHMDA law. This may include:
- Financial Assistance Information: Information you choose to share about your potential eligibility for Patient Assistance Programs (PAPs), such as general income or insurance status.
- Medication Information: Data regarding prescription medications or treatments you are seeking support for through our medication navigation services.
- Healthcare Barriers: Information collected via secure intake forms regarding your healthcare barriers and medication access challenges.
Currently: Our public website only collects basic contact information (such as name, email, and organization) and general inquiries. We request that you do not include diagnosis, treatment history, or other detailed health information in contact forms.
2. Sources of Collection
We collect this data directly from you when you:
- Submit an inquiry through our contact page
- (In the future) Engage with our navigation team through secure channels once services are live
- Use the Weaver Crest platform for medication navigation assistance (launching Q3-Q4 2026)
3. Purpose of Consumer Health Data Collection
We collect and use Consumer Health Data solely to:
- Determine your eligibility for financial assistance programs
- Provide the medication navigation services you have requested
- Comply with CMS billing requirements for patient navigation codes (G0023, G0019)
Your Data Is Protected:
- We never sell your health data
- No targeted ads using your data
- Used only for your requested services
4. Sharing of Consumer Health Data
We may share your data with:
- Service Processors: Service providers who help us operate our secure platform (for example, hosting, analytics, email, and workspace tools), strictly for the purpose of providing our services on our behalf under HIPAA Business Associate Agreements.
- Fiscal Sponsor: Evergreen Social Impact, to the extent necessary for fiscal sponsorship, compliance, and auditing purposes under our fiscal sponsorship agreement.
5. Your Consumer Health Data Privacy Rights
Under Washington State MHMDA law, you have the right to:
- Access: Confirm whether we are collecting your data and access that data
- Withdraw Consent: Revoke your consent for us to collect or share your data at any time
- Delete: Request that we delete your Consumer Health Data
